A caregiver failed to demonstrate competency and good judgment by physically intervening when a child threw a tantrum.

A caregiver did not have a current certificate of training in pediatric first aid with rescue breathing and choking. The caregiver's last training was taken on 7/30/22 and expired on 7/30/24.

A child in care was left unattended in the outdoor play area of the operation.

During the investigation it was determined a child was left outside on the playground unattended for no more than five minutes.

An incident of prohibited punishment involving a child in care occurred on 05/01/23, this incident was witnessed by a caregiver and not reported to the director. A second incident of prohibited punishment involving another child in care occurred on 05/02/23, this was reported to the director on the same day. The incident report was not provided to the parent within 48 hours.

It is alleged that several children in care were inappropriately disciplined.

Ten out of ten employee files did not have the required trainings that focus on the following training topics. (1) Emergency preparedness; (2) Preventing and controlling the spread of communicable diseases, including immunizations; (3) Administering medication, (4) Preventing and responding to emergencies due to food or an allergic reactions; (5) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (6) Handling, storing, and disposing of hazardous materials.
